NEW STUDENTS:

Preschool/PDO: IL Physical Form

Kindergarten – 8th grade: IL Physical, Dental and Vision Forms

 RE-ENROLLING STUDENTS:

Preschool: none

Kindergarten: IL Physical, Dental and Vision Forms

1st grade: none

2nd grade: IL dental form

3rd grade: none

4th grade: none (Sports Physical if participating in sports at St. Peter)

5th grade: none (Sports Physical if participating in sports at St. Peter)

6th grade: IL Physical and Dental Forms

7th grade: none (Sports Physical if participating in sports at St. Peter)

8th grade: none (Sports Physical if participating in sports at St. Peter)

If you would like your child to be able to take an over-the-counter medication during school (like Tylenol or Advil) the state requires a doctor authorization. Please complete the OTC Medication Authorization Form, which needs to be signed by your child’s doctor. If your child takes daily prescription medication, please contact the school nurse for a special form. All medication needs to be given through the school nurse. Students may not take medication on their own.

 All forms are due on Final Registration Day on August 9th. All forms are available on our website at stpetermyschool.com.
